If you would like to request access to medical records, please download, print and complete the 'Request for access to medical records' form.
The form requires a signature and should be returned to:

Cost of obtaining access to medical records
If you want a copy of the information in your records there is a standard administration charge of £10.00, together with a copying charge of 25p per sheet.
